Most "anti-wrinkle pillows" are a flat pillow with a hole cut in it.

They photograph beautifully. They all promise the same thing. And from a product page, at $250 to $300 a piece, there is almost no way to tell which ones actually do anything — so here are the five questions that separate them.

A pretty product photo doesn't tell you how it sleeps.

Every beauty pillow is shot the same way: soft light, white bedding, a model who is definitively not asleep. What the photo cannot show you is the only thing that matters — whether your face is genuinely held clear of the surface, or quietly compressed against it for eight hours.

It cannot show you how the shape supports your head under real weight. Whether the height adjusts to your neck, or was decided in a factory. How many positions does it actually work in?

One cut-out means one correct position, which means it stops working the moment you turn over. Ask how many sides of the pillow your face can land on and still be clear of the surface.

Can you change the height?

Neck comfort is personal and a pillow you can't adjust is a gamble. A fixed loft decided in a factory either suits you or it goes back. Look for a fill you can add to or remove.

Does it address friction as well as pressure?

These are two different problems. A contoured shape addresses pressure. A satin or silk surface addresses friction. A pillow that only addresses one leaves the other running all night — and most sell the case separately.

Is the shape protected — or just copied?

A patent is a reasonable proxy for whether real design work happened. Unprotected shapes get cloned within a season, which tells you how much thinking went into the original.
